You will find below contact information for staff athletic trainers, information regarding physicals and how to complete the yearly consent forms required by the UIL and Athens ISD. RankOne Sport is the mandatory format for submitting yearly consent forms. There is no need to create an account on the RankOne Sport parent portal (link below), but you will need your student’s ID number.

There is a page for emergency contact information and a page for UIL consent forms. Both of these pages are required for sports participation and must be completed before an athlete can be cleared to play. This is in addition to the athletic physical required for any student to practice or play in games. (More on that below.)

Pre-participation requirements

Athletic Physicals

All incoming seventh-grade student athletes and anyone else who has never received a physical for sports participation is required to get a physical from a MD, NP or PA. This is a more thorough physical exam and is mandatory for these athletes.  AISD conducts mass physicals at the high school field house during the spring for students who already have a physical on file, need a new one, and are planning to compete the next school year. Athletic physicals are valid for two years after they are completed and turned into the athletic trainer.

UIL and Athens ISD Consent Forms

The consent forms required by Athens ISD and the UIL are all completed online through RankOne Sport (see the link above). These must be completed every year prior to a student's participation in any school-related athletic practices or games.
