Public Complaints

Below each Level will be the appropriate Complaint Form.


Level 1 should be selected when:  complaint_form_level_1
First notification to the campus in question.
Please print the form, complete all blanks and turn it in to the campus principal.


Level 2 should be selected when:  complaint_form_level_2
Any person appealing a level one decision
Please print the form, complete all blanks and turn it in to the Superintendent or designee.


Level 3 should be selected when:   complaint_form_level_3
Any person appealing a level two decision
Please print the form, complete all blanks and turn it in to the Superintendents office requesting it be passed to the Board in accordance with Board Policy GF: (LOCAL).
